Fazia is a 52-year-old woman who has been taking regular Ibuprofen tablets for her rheumatoid arthritis.
Fazia has been experiencing abdominal discomfort for the past 24 hours but has continued taking her over
the counter ibuprofen to ease the pain. This morning Fazia experienced three episodes of vomiting fresh
blood.
Fazia is brought into the Medical Assessment Unit by Ambulance with a provisional diagnosis of a gastro
intestinal bleeding.
You go to see Faiza and she appears pale and looks drained and lethargic. She complains of
epigastric pain and feeling dizzy. When you check her vital signs, you find the following parameters:
• Respiratory rate: 24 breaths per minute
• Oxygen Saturations: 90% on room air
• Pulse: 120 beats per minute
• Blood pressure: 85/53 mmHg
• Capillary refill time: 3 seconds
• Temperature: 35.4 degrees centigrade

Question 2
Monty is a 44-year-old man who has a respiratory tract infection and was commenced on oral
antibiotics four days ago. He calls his GP today as he feels his condition is worse. The GP is very
concerned about his breathless condition and refers him to the medical assessment unit.
On arrival he feels very unwell, unable to speak in full sentences, looks flushed and is shivering.
Monty is diagnosed with community acquired pneumonia. His vital signs are as follows:
• Respiratory rate 28 breaths per minute
• Oxygen saturations 89% on room air
• Pulse 125 beats per minute
• Blood pressure: 87/40 mmHg
• Capillary Refill Time: 4 seconds
• Temperature: 38.7 degrees centigrade

Instructions:
Critically discuss, with reference to the relevant pathophysiology, the nursing assessment, priorities of care
and nursing management of this acutely unwell patient for the first 24 hours following admission.
Your answer should also demonstrate your underpinning knowledge of relevant pharmacology, psychosocial
aspects as well as any potential complications.
